Medicare Select HMO

Thank you for your interest in Medicare Select (HMO). Our plan is offered by Denver Health Medical Plan, Inc. (DHMP) which is a Medicare Advantage Prescription Drug Plan (MA PD).

You must live in Denver County, you must be eligible for Medicare Part A and enrolled in Medicare Part B, you must be eligible for full Medicaid health benefits and not have End Stage Renal Disease (ESRD).

Premiums, copays, coinsurance and deductibles may vary based on the level of 'Extra Help' that you may receive. You should contact Medicare Choice for further details. Medicare Select Plan Information

Plan Premium: $32.40

Plan Part B Deductible: $ 140.00

Medicare Select Enhanced Benefits

These benefits are available to Medicare Select members at no additional charge,
subject to the limits noted.

Dental Services: You are eligible for up to $1,850 for preventative and comprehensive services through Delta Dental every year.

Hearing Services: You are eligible for up to $1,500 for hearing aids every year. You will have 0% or 0% - 20% of the cost for each Medicare-covered diagnostic hearing exam.

Vision Services: You are eligible for up to $200 for eyewear every year. You will have 0% or 20% of the cost for exams to diagnose and treat diseases and conditions of the eye.

Transportation: $0 copay for each round trip, up to 10 trips each year to Plan-approved locations.

Routine Podiatry Care: 0% coinsurance for up to four routine Podiatry visits every year.

Preventative Screening: $0 copay

Health Club Membership: Free health club membership to any Denver Park and Recreation Center for a year.

NOTE: Obesity Therapy Effective November 29, 2011, Centers for Medicare Services implemented an Obesity Therapy benefit for Medicare members. Expenses incurred for these services beginning 11/29/11 will be reimbursed through Denver Health Medical Plan, Inc. for both Medicare Choice and Medicare Select.
