Care Management at Denver Health
Care Management provides both Health and Wellness Coaching and Disease Management services to any DHMP member including Plan A, Point-of-Service, Medicare Select, Medicare Choice and CHP+. We are also available for those members with Medicaid Choice.
There are several types of Care Management services available - all are free and voluntary to members. These include:
- Face to Face Health Coaching in our Community Health Clinics
- Assessment and Intervention
- Telephonic Coaching and Support
- Member Health and Education
- Information and Community Resources
- Health and Wellness Education Series
- Support Groups
- Chronic Disease Self-Management Workshop Series
- Group visits focused on Medical and Behavioral Health Issues
Care Management services help members to:
- Make lifestyle changes and reduce risks
- Take action by making short and long term goals to improve health
- Promote wellness and improved health status
- Facilitate self management of health and disease states
Care Management staff also:
- Advocate for members to ensure access to health and social services
- Encourage appropriate utilization of care
