Health and Wellness Services

The Health and Wellness Services Department encompasses the Health Coaching program as well as a number of grant-funded initiatives and research projects focused on innovative disease management and risk reduction interventions.   The goal of the department is to support the patient-provider relationship and plan of care, utilizing evidence-based practice guidelines and patient empowerment (self-management) strategies, such that members are encouraged and supported to actively participate in improving their health status. 

 

The department is focused engages in ongoing development and evaluation of population-based, chronic care improvement programs and health promotion interventions which include, but are not limited to:  health coaching, integrated behavioral health care, use of Interactive Voice Response (IVR) technology, and use of interactive web-based programs that help members develop skills to make healthy choices and adopt lifestyle interventions to effectively self-manage medical and behavioral health conditions.

 

Health Coaching is available for DHMP and Medicaid Choice members with chronic disease(s) such as asthma, diabetes (DM), chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), and hypertension (HTN).  Health Coaching also includes preventive health measures for alcohol, tobacco and other substance use as well as increasing physical activity and weight loss. 

 

The Telephonic Depression Intervention is a program sponsored by the Robert Wood Johnson Foundation and offers telephonic counseling for depression to qualified patients to assist primary care providers improve the management of depression treated in the primary care setting
