Effective self-management allows patients to have a vital role in determining their care and promotes a sense of self-responsibility for health and well-being. The Care Management program is offering a new collaborative and solution-focused self-management group (click on the link above to view the patient flyer). A dialogue and an exchange of ideas between group members is encouraged and focuses on defining problems/barriers, setting priorities, establishing goals, and identifying solutions such that patients feel empowered to take a more active role in their treatment.
What: FREE Health and Wellness Self-Management Group (English only)
When: Every Friday from 9:00 – 10:30 (beginning July 9th)
Where: Westside Clinic - Rose Chavez Conference Room
Who: All patients are welcome to attend (however space is limited and priority will be given to Managed Care patients)
Facilitator: Rachael Meir, PsyD
Topics discussed in the group sessions may include, but are not limited to:
- Healthy eating
- Weight management
- Medication adherence
- Coping with stress
- Physical activity
- Reducing unhealthy habits
- Sleep hygiene
